Welcome to Sother Divine Care's Adult Foster Care Program

Sother Divine Care, LLC provides compassionate Adult Foster Care support for eligible adults who need daily assistance in a safe and supportive home setting. Our program helps participants receive the care they need while remaining connected to the comfort of home, family, and community.

Our team works closely with caregivers and families to promote dignity, safety, comfort, and quality of life through ongoing care management, nursing oversight, and caregiver support.

What is Adult Foster Care?

Adult Foster Care, also known as AFC, is a MassHealth-supported program for eligible individuals who need daily assistance with personal care and activities of daily living. Services are provided in a private home setting by a qualified caregiver who lives with the participant and supports their daily needs.

AFC may be appropriate for adults who need help with activities such as bathing, dressing, toileting, mobility, eating, and personal care, but who do not require continuous skilled nursing care.

Through Adult Foster Care, participants receive support in a familiar home environment while caregivers receive guidance, training, and ongoing support from the care team.

Our Services

Sother Divine Care provides Adult Foster Care services in accordance with Massachusetts AFC program requirements. Services are individualized based on each participant’s assessed needs, care plan, and home environment.

  • Personal Care Assistance:
    Support with daily living activities, including bathing, dressing, grooming, toileting, eating, mobility, and personal care.
  • Care Management and Oversight:
    Ongoing support from qualified staff, including care managers and registered nurses, to help monitor the participant’s health, safety, and care plan needs.
  • Medication Support:
    Guidance and oversight to help ensure medications are taken as prescribed. Adult Foster Care does not provide continuous skilled nursing or skilled
    medication administration.
  • Nutritional Support:
    Support with meal planning and preparation in collaboration with the caregiver, based on the participant’s dietary needs and preferences.
  • Caregiver Support:
    Training, supervision, and ongoing guidance for caregivers to help promote quality care and compliance with AFC program standards.
  • Companionship and Social Support:
    Encouragement of social engagement, emotional support, and meaningful daily routines to promote overall well-being.

Adults may qualify for Adult Foster Care through MassHealth based on their care needs, coverage, and ability to live safely at home with caregiver support.

  • Has MassHealth eligibility for AFC services
  • Needs help with daily living activities
  • Has a medical, physical, cognitive, or mental health condition
  • Can live safely in a private home with a qualified caregiver
  • Does not need continuous skilled nursing care

How to Enroll

  1. Contact Us:
    Share the participant’s needs and caregiver situation.
  2. Eligibility Review:
    We review MassHealth coverage and basic care needs.
  3. Assessment:
    Qualified staff assess the participant’s support needs.
  4. Care Plan:
    A care plan is created based on the assessment.
  5. Caregiver Support:
    The caregiver receives training and guidance.
  6. Ongoing Oversight:
    Nurses and care managers provide continued support.
