What is Adult Foster Care?
Adult Foster Care (AFC) is a MassHealth-funded program that provides personal care and support services to adults who are unable to live independently due to medical or mental health conditions. Services are delivered in a private home setting, allowing individuals to receive care in a safe and familiar environment. This program is designed for individuals who require assistance with activities of daily living (ADLs) such as bathing, dressing, mobility, and personal care, but who do not require continuous skilled nursing care
Our Services
Sother Divine Care provides care management and support services in accordance with Massachusetts AFC program requirements. Services are individualized based on each participant’s assessed needs.
- Personal Care Assistance:
Support with daily living activities, including bathing, dressing, grooming, toileting, and mobility. - Care Management and Oversight:
Ongoing monitoring by qualified staff, including registered nurses and care managers, to ensure the participant’s health, safety, and care plan needs are met. - Medication Support:
Guidance and oversight to help ensure medications are taken as prescribed. AFC does not provide skilled medication administration. - Nutritional Support:
Assistance with meal planning and preparation in collaboration with the caregiver, based on the participant’s dietary needs. - Caregiver Support:
Training, supervision, and ongoing support for caregivers to ensure quality care and compliance with program standards. - Companionship and Social Support:
Encouragement of social engagement and emotional support to promote overall well-being.
